Welcome to Bayside OB-GYN, we are now accepting new patients. You can call the number to the right to contact us and schedule an appointment.
Click the links below to download the forms which can be printed and filled out before you arrive. This is the best way to get the most out of your first visit with us, but we also have copies of these in the office if you cannot complete them before you arrive.
General forms:

- Statement of Financial Responsibility / Assignment of Benefits
If you are pregnant, please also complete the following three forms:
- New OB Insurance & Patient Responsibility Form
- Prenatal Genetic Screen
- Women and Infants Preadmission Form
Additional helpful information
Please bring your insurance card and a picture ID to all of your visits.
Check with your insurance company regarding the benefits that your plan offers and obtain any necessary referrals prior to your appointment. Due to our contractual obligations with your insurance provider, we are required to collect your co-pay at the time of your appointment.
Please bring any copies of records from other providers you feel may help with your treatment.
